Formal letter. (pismo oficjalne) Daje naj!!!

Dear Sir or Madam I am writing to report a fault with the DVD that I bought from your shop in Ostrowiec Świętokrzyski on 12 th January . It works very good when I watch two first films but when I try on next film- Fast 6 it turns off. I do not know what happened. DVD is domage and it is not works. I am  returning the DVD  to you with this letter. I would be grateful if you could repair the fault or send me a new DVD. I really  depend on my time becouse I invite my friends for film on Saturday . I look forward  to hearing from you. Yours faithfully. xyz
